At Home Caregivers, LLC we use training programs developed by the Institute for Professional Care Education® to ensure that every employee has the skills and knowledge needed to provide high quality care.


We pride ourselves on hiring people for their compassion, experience, and flexibility. But we know this isn’t all it takes to provide the best care possible. Caregivers also need excellent skills and competency in care — and that comes from excellent training.

Personal Health Supporters Program™

The Personal Health Supporters Program is an optional program that places a strong emphasis on the progression of care with setting individualized goals, putting a plan to action, and evaluating the outcomes. It allows you to stay with the basic program or choose a stronger, more interactive approach with your caregiver.

Care plan goals will be individualized for each client during a free, complimentary RN assessment. Home Caregivers will then formulate your own personalized health goals that we would support by encouragement and assistance from your caregiver.

A strong emphasis is placed on addressing specific disease processes and conditions by a specially-trained caregiver for CHF, Diabetes, COPD, Dementia, Parkinson’s, Mental health, Post-surgical/Post-partum, and Pediatric Care.

Expected Outcome

Mobility/Activity is maintained at optimal level
  • Encourage independent activity per ability.
  • Maintain proper body alignment
  • Perform active ROM exercises.
  • Increase activities as tolerated – Ambulate around house 2X/day
  • Plan activities to conserve energy: plan rest periods.
  • Reposition every 2 hours

Educational materials will be presented and provided to you on the first visit. Suggestions or recommendations will be made for clients and families if goals are not being met. Goals can be modified at any time. You can opt out of the program at any time.

Web-based Family Portal designed for family members to be indirectly involved in your loved one’s care.
Family members are able to communicate with staff, see their loved one’s schedule, view invoices, and even make payments for services.
